Belt Lipectomy

The procedure is an extension of the high lateral tension tummy tuck concept developed by Ted Lockwood. Lockwood basically re-invented the tummy tuck by re-defining the nature of suspension of the deeper tissues of the abdomen and flank. The lower body lift extends the tummy tuck incision completely around the lower torso. This allows re-suspension of the lateral and anterior thighs along with the traditional tummy tuck improvements.

The operation is extensive and is known for wound healing complications. Some surgeons have arbitrarily divided the operation into smaller portions to help limit these complications. Furthermore, the concept has been expanded upon to include additional lifts such as those of the inner thigh (medial thigh lift). The traditional lower body lift targeted the upper thighs as well as the buttocks and abdomen. For the large volume weight loss patient, this operation is ideal to remove the redundant skin that frequently "hangs" around the lower torso.

These operations typically require 5–8 hours operating time when done together. The procedure requires time off from work. Close post-operative supervision is advised.

What to Expect After Surgery

A belt lipectomy procedure takes about four to five hours to perform. Most people who undergo the surgery need to spend at least one night in the hospital. This procedure, like most skin reduction surgeries, such as an abdominoplasty, thighplasty, arm lift (brachioplasty), or breast lift (mastopexy), is a major operation that requires significant recovery time. The recovery time differs from person to person. Most people need about two to three weeks before they can return to light activity. Usually, people will need about two months before they feel completely recovered. Of course, if there are complications, it can take longer before healing is complete. The complex nature of this type of surgery requires that your surgeon be intimately familiar with the belt lipectomy procedure, bariatric surgery, and the nutritional requirements of patients who undergo these procedures. During recovery from a belt lipectomy, a binder will need to be worn around the belly and back for about six weeks. It is often helpful to have someone to assist you perform day-to-day tasks during the first few weeks after your surgery. Immediately following a belt lipectomy, however, you will be able to walk, lie down, and sit for brief periods of time.

It is important to remember that the only way to remove skin is by making incisions. Therefore, while you will be trading loose skin and fat for a wonderful shape and firm, tight skin, there will be some scarring after the incisions heal. The incision for a belt lipectomy or circumferential body lift, as performed by Ohio surgeon Bivik Shah of the Columbus Institute of Plastic Surgery, is around the circumference of the lower body near the beltline. An incision is also sometimes made straight up and down in the middle front of the belly. Some patients already have an up and down scar there as a result of their gastric bypass operation.


Typically general anesthesia but spinal anesthesia is also an option.


Because of the extensive nature of the procedure an overnight stay is usually required.

Possible Side Effects

Breakdown of the suture line above the buttocks (because of patient's tendency to bend at the waist when performing normal hygiene which strains the suture closure pulling it apart). This usually leads to a wider scar in the posterior surgical area. Some numbness of the area above and sometimes below the incisions, residual looseness and irregularity of the skin, and all the usual common side effects of any surgical procedure.


Risks of surgery are the same as with conventional abdominoplasty.


Recovery is prolonged because of the requirement of avoiding bending at the waist for a month until adequate healing has occurred.
